The report forms,  which are to be submitted annually, are now available on the Tenarky web site at www.tenarky.org  These forms are due by January 15, 2009.  Please fill out the appropriate form as soon as your activities for 2008 are completed and send to the appropriate Tenarky Chairman. 

This is an important requirement.  No one can be considered for an award unless they have submitted the proper form.
